What is estradiol therapy?

Estradiol is the main form of estrogen your body makes before menopause. When levels fall, many women get hot flashes, night sweats, broken sleep and vaginal dryness. Replacing some of that estrogen is the most effective treatment for hot flashes and night sweats.

Patches, gels and tablets treat symptoms throughout the body. A low-dose vaginal cream works mainly where it is applied, for dryness and discomfort. Women who still have a uterus also take progesterone to protect its lining.

Who it may be prescribed for

  • Women with moderate to severe hot flashes or night sweats
  • Women with vaginal dryness or discomfort during sex
  • Healthy women under 60, or within ten years of their last period

Do I also need progesterone?

If you still have a uterus and use estrogen that acts throughout the body, yes: progesterone protects the uterine lining. Low-dose vaginal estrogen usually does not need it.

Which form should I choose?

Patches and gels avoid the liver and may carry a lower clot risk than tablets. Your provider will help you weigh the options.

Important safety information

Estrogen therapy is not right for everyone. Estrogen taken without a progestogen raises the risk of cancer of the uterine lining in women who still have a uterus. Estrogen, with or without a progestogen, can raise the risk of blood clots, stroke and gallbladder disease, and long-term combined use is linked with a small increase in breast cancer risk. In a large study of women 65 and older, hormone therapy was linked with a higher risk of dementia.

Do not use estrogen if you have had breast or uterine cancer, blood clots, a stroke or heart attack, liver disease or unexplained vaginal bleeding, or if you are pregnant. Your provider reviews your personal and family history first and prescribes the lowest dose that helps.
